Tools are available for 2025/26. At time of writing the Clinical Systems team is only aware this ES is active in East Suffolk; therefore tools have been provided only for SystmOne sugeries.

Reports have been provided to show items that require action.

Quarterly submission reports

These reports must be run in July 2025, October 2025, January 2026, and April 2026 as they are relative to end of month.


Data will be supplied to the Primary Care team for the following items. Items 1-6 of the ES submission must be managed by the surgery.

  • 07a) Falls risk recorded (FRAT score or 'At low/high risk of falls') or declined
  • 07b) FRAT 3+ OR 'At high risk of falls'
  • 07c) FRAT 3+ OR 'At high risk of falls' AND PCSP (or declined)
  • 08a) Without dementia
  • 08b) Without dementia with a memory/dementia assessment (or declined)
  • 09) With ReSPECT form
  • 10) With Structured medication review (or declined)

Recording/Data entry

Surgeries can use any entry methods that provide the necessary data; however, the reports have been built with the intent of using the Ardens tools.

  • Care Home Residents: Nursing & Residential Homes
    All other templates are accessible from the 'Care Home Residents' template on the 'Review Visit' page:
    • Falls and/or Falls Risk Assessment Tool Score (FRAT Score)
    • Future Care Planning (For ReSPECT)
    • Drug Review
    • Personalised Care & Support Plan PCSP (PCSP)

Note:

  1. Entries in 'Falls risks' field (below) will not achieve the requirements of the LES. Entry must indicate high/low risk of falls via code or numeric FRAT Score (see below)
  2. Generic entries of medication review will not achieve. Structured medication review must be recorded.

Record a FRAT score (0-5) and/or clinical Impression of high/low risk

FRAT Score template is accesible as part of the patient Review Visit (see above), as well as a link to a more detailed Falls assessment.
