AF008 - Why isn't Warfarin working?

This article is a summary of QOF AF008 (as at Feb 2025) and is not prescribing advice.


By default, since April 2023 patients are expected to primarily be on a direct-acting oral anticoagulant (DOAC), not a Vitamin K antagonist such as Warfarin. Issuing Warfarin will not achieve this indicator without additional action.

Issuing a DOAC

No additional clinical justification or entries are required to achieve this QOF indicator as long as the patient has not had a Mechanical valve replacement.

Issuing a Vitamin K antagonist (e.g. Warfarin)

To achieve/exempt patient from this QOF indicator a code indicating the following is required:

Indefinite

Patient has...

  • A DOAC allergy (Exempt)
  • Had a Mechanical valve replacement (Achieve)
  • Antiphospholipid syndrome (Achieve)

Expiring

  • Patient declined DOAC in last 12 months (Achieve)
  • INR time in therapeutic range* is 65%+ in last 6 months -AND- Clinician determined DOAC not indicated in last 12 months (Achieve)


*This can be calculated with 'Warfarin Monitoring Data' on the Clinical Tools menu; but the numeric value must be manually added separately.
