The Clinical Systems Team recommend:

1. All surgeries are active with the 'Register with a GP Surgery Service'.
   This allows patients to complete a standard registration via NHS.uk, the NHS App and your website (where you provide a link). This registration method performs validation on contact details and checks the current registration status, reducing links rejections and surgery workload.

2. Use the Ardens 'New Patient Registration' template as your default registration template.
   The template is updated based on requirements and recommendations for new patients.
   To set as your default registration template: Setup > Users & Policy > Organisation Preferences > Registration > Miscellaneous. Change the option for When a patient is registered, display this template. New Patient Registration can be  selected from category Ardens Ltd / Information.
   
   
3. Use the Ardens paper registration forms.
   Links to these forms are updated based on requirements and recommendations for new patients.
   Do not print large quantities or save local copies as these will not be updated. Add the links to these forms to your website for patients as a secondary option after the link to the online option for 'Register with a GP Surgery Service'. Where you wish to print copies do so from the same links so you hold the latest version.

Each patient should have a completed registration form and a health questionnaire.

1. GMS1 (Registration form)
2. Health questionnaire (based on age group): Adult / Child / Newborn

If you are an existing NHS patient, please aim to include your NHS Number on your forms; you can get this with the Find my NHS Number service, from your previous GP surgery or on NHS letters or prescriptions you already have.
