If these issues are not resolved Patient Services will not be notified you have accepted a patient and they will remain a patient of their previous surgery.

No town in previous address
This occurs when the address from the patient’s previous surgery does not have details in the correct fields and therefore the Town field has been left blank. This isn’t obvious when checking the record as you may even be able to see that the town is present, but without further investigation you cannot see it is recorded in the wrong place.
The task will appear as below.
- Right-click and Retrieve Patient
- Navigate to the Administrative tree, Address History node and then Address History tab.
You will see the past addresses available. The incorrect entry likely has no details in the Recorded By column as this data was imported from another clinical system. - Click the Record New Address button and select Home
- Important: Select This is a past address
- Double-click the past address listed on the right-hand side (Previous Addresses tab) which you believe to be incorrect. If you are unsure: double-click each entry in sequence, the details will move to the left-hand side of the screen. One will have no details in the Town field.
- Click Find next to the Postcode field and click Ok to enter this corrected address.
If the Postcode is valid SystmOne automatically corrects the address when you click 'Find’. If it is invalid or missing you can ask the patient for the correct details or try to locate it using the Royal Mail Postcode Finder. http://www.royalmail.com/find-a-postcode.
- There will now be duplicates of the address. Right-click the original incorrect entry and Mark in Error.
- Select an appropriate entry and type a reason.
- Save the patient record.
- Right-click and Action the task.
Invalid characters
These errors are much easier to find in the past address list as you can see the erroneous entry. There will be symbols such as: ; : & / ) (
To resolve this you need to re-record the past address and mark the incorrect entry in error. Ensure you do not use the specified symbols. i.e. Exchange & for and
