Bulk application of sharing decisions after migration

My Care Record (MCR) is a regional policy to share care records by default. Any questions regarding the MCR policy should be directed to your surgery Data Protection Officer or the contact details on the MCR website: https://mycarerecord.org.uk/

Technical queries can be raised with the Clinical Systems team by submitting a ticket.


EMIS Web and TPP SystmOne record sharing preferences differently so after migration action must be taken to ensure patient preferences are honoured and that their record is shared or not shared accordingly.


If your surgery was migrated to SystmOne by the ICB Clinical Systems team, the following action should have been completed for you. If you were migrated by a different team or this was not completed, use the following to complete for any remaining patients.

Part 1: Explicit dissent

This must be completed first.

You will identity patient records in EMIS Web that explicted dissented to sharing their full record and then record this in SystmOne.


These instructions assume familiarity with EMIS Popluation Reporting and navigating TPP SystmOne.

  1. In EMIS Web:
    1. Locate the search My Care Record: Explicit opt out in Population Reporting.
    2. Copy and Paste into your organisation searches.
    3. Run the search and view the included population - Take note of the patients in your preferred method
      (e.g. Refer to on screen/Print/Export to excel)
  2. Within SystmOne
    1. Retrieve the first patient
    2. Navigate to Administrative (Tree), Record Sharing (node), right-click the node and select Record Sharing
    3. Under Sharing Out select No - do not share any data recorded here
      Under Sharing In select Consent refused
      It should look like this:
    4. Click Ok and Save the record
    5. Repeat these steps for each patient in the search from EMIS.

Part 2: Wait!

You must now wait until the SystmOne reporting build is including the dissent entries you have added. This typically means the next working day; however, if you entered any data (above) after around 5pm this may require another working day.

Part 3: Bulk-recording implied consent

Check that your explicit dissent patients are showing in SystmOne Clinical Reporting.

  1. Navigate to Reporting > Clinical Reporting
  2. Access the categories: All Reports > By Owner > System Wide > Sharing consents
  3. Highlight the 3 indicator reports and click Run ▶.
  4. When these have run, right-click Explicit Sharing out: Patients with explicit dissent and select Show Patients (magifying glass).
  5. Check all the patients you entered dissents for are listed (there may also be other patients that have dissent recorded by a prior SystmOne surgery).
    If any are missing, right-click the name, Retrieve Patient and check the Record Sharing node shows the dissent for sharing out. If not, re-record. If it is, you likely recording this after the current reporting build and need to wait until the next reporting build.
  6. Repeat the steps above for Explicited Sharing in: Patients with explicit dissent, confirming all the patients from Part 1 are listed.

Now that you know Clinical Reporting is aware of the patients with explicit dissent, we can bulk-record implied consent on the remaining patients.

Note: Bulk changes to records can take considerable time to process; you will not be able to use the system while this runs. (We recommend tea. ?)

  1. Right-click the report: Actions > Update Record Sharing Preferences
  2. Sharing Out: Select Yes -share record > Policy with reason www.mycarerecord.org.uk
    Sharing In: Select View shared record > Policy with reason www.mycarerecord.org.uk
    It should look like this:
  3. Click Ok to start, You will be asked if you are sure you wish to proceed, select Yes.
    (The warning are you are agreeing to is imported where reports arenot already checking for and excluding those with existing share preference; but this report is making those checks)

The system will now apply these implied decisions to all patients in the report. When finished you will be able to use SystmOne again.


Recommended: Return this this report after the next reporting build and re-run to confirm there are no remaining patients.


Part 4: Implied sharing preference for new patients

  1. Ensure that your privacy note/policy and new patient information media (website/registration pack) include information about My Care Record and links to the website. This gives patients an opportunity to advise if they wish to opt out.
    Optional note: My Care Record is an "opt out" model that relies on implied consent; but you can choose to offer new patients an explicit decision in your registration process and record their consent or dissent to sharing out and sharing in.

  2. Ensure your SystmOne unit is configured to automatically apply implied sharing decisions to new patients.
    1. In SystmOne, open Organisation Preferences (main menu: Setup > Users & Policy)
    2. Navigate to category and sub-category: Clinical Policy > Sharing
    3. Sharing Out: Tick Automatically record sharing out preference
      Set as Yes - share record > Policy with reason www.mycarerecord.org.uk
      Sharing In: Tick Automatically record sharing in preference
      Set as View shared record > Policy with reason www.mycarerecord.org.uk
      It should look like this:
    4. Click Ok
