Your Care ID profile will have your legal name registered as per documentation you submit to prove or update your identity. This is stored in field Full name which is name the applications will use.
Change of name
If you've changed name, please submit documentation to evidence this.
Acceptable documentation
- Deed poll (see below)
- Passport
- Driving licence
- Marriage/civil partnership certificate
- Decree absolute/civil partnership dissolution certificate
Deed polls
If you have changed your name, but have not yet arranged to update items like passport/driving licence, we recommend a deed poll.
You can write your own. This is called an unenrolled deed poll and has equal legal validity to an enrolled deed poll; the latter incurs a charge and is published in public records by the High Court).

Name on smartcard
Smartcards are the only authenticator type that include a photo and name. That the name printed on the card does not need to be the Full name. This can be a reasonable derivative based on your preference. If you do not specify otherwise the printed name will be in format: Forename Surname
