Names and Change of Name

Your Care ID profile has your legal name registered in line with documentation used to prove your identity. This is stored in field Full name and is also the name that will display in most clinical or administrative applications.

Name on smartcard

Smartcards are the only authenticator type that include a photo and name. The printed name does not need to be the Full name on your Care ID profile. This can be a reasonable derivative based on your preference. If you do not specify otherwise the printed name will be in format: Forename Surname


Change of name

If you've changed name, please submit documentation to evidence this.

Acceptable documentation

  • Deed poll (see below)
  • Passport
  • Driving licence
  • Marriage/civil partnership certificate
  • Decree absolute/civil partnership dissolution certificate

Deed polls

If you have changed your name, but have not yet arranged to update items like passport/driving licence, we recommend a deed poll.

You can write your own deed poll, which is an unenrolled deed poll. It has equal legal validity to an enrolled deed poll; however, the latter incurs a charge and is published in public records by the High Court. Note that some organisations may not accept an unenrolled deed poll as proof of name change.
